Please provide the following information as part of your application:
- Full Name and Contact Information
- Breeder Type (USDA Licensed or Hobby)
- If USDA Licensed: Include your current USDA License Number and any previous license numbers associated with your name or address
- If Hobby Breeder: Please list the number of dogs currently in your care
- Breeds you offer
- Veterinary Reference (Name and Contact Info)
